Payment is due at the time of your visit.  A service charge of $10.00
will be added to help cover accounting costs if you choose not to pay
at the time.  It is the patient's responsibility to know his/her
insurance benefits.  Preventive medicine, physical exams, pap smears,
immunizations, etc. are not covered by most insurance policies.
Benefits information can be obtained from your insurance company or
through your employer's personnel department. Please keep our office
updated with changes in your insurance policy.  …In most cases as a
courtesy to our company hospitalized patients, our physician's fees
for hospital services are billed directly to your insurance company.
If your insurance does not pay in a timely manner, we will expect
payment from you, unless other arrangements are made with the office.
After your release from the hospital, the follow-up office visits not
covered under many policies; therefore payment is expected at the time
